Understanding Aneurysmal Bone Cyst (ABC): Causes, Symptoms, and Treatment

Aneurysmal Bone Cyst (ABC) is a rare, non-cancerous bone tumor. It most commonly affects children and young adults under the age of 20. Although it is benign, it can be quite aggressive locally.

What is an ABC?

An ABC is not a true cancer. It is a hollow, blood-filled sac or cyst that develops inside a bone. As it grows, it causes the bone to expand and become thin, resembling an aneurysm in a blood vessel. It usually develops in the long bones of the legs and arms, or in the spine.

Common Symptoms

  • Pain and Swelling: The most frequent symptom is localized pain that worsens over time.
  • Mass: A visible or feelable lump may develop near the affected area.
  • Stiffness: Limited range of motion in nearby joints.
  • Pathological Fracture: The bone can become so weak that it breaks under normal stress.

Diagnosis

First, doctors diagnose ABCs using imaging tests like X-rays, which show a classic “soap bubble” appearance. Furthermore, medical teams order MRI or CT scans to clearly see fluid levels inside the cyst. Finally, pathologists always perform a biopsy to confirm that a malignant sarcoma is not causing the tumor.

Treatment Options

  • Curettage and Grafting: The surgeon scrapes out the cyst and fills the hole with a bone graft or cement.
  • Sclerotherapy: Injecting a special medication directly into the cyst to make it collapse and heal.
  • Targeted Therapy: Modern treatments sometimes use medications like Denosumab for complex cases.

The outlook is generally excellent after treatment, though regular follow-ups are needed because these cysts can sometimes return.
