Finkelstein Test Demonstration Steps

Doctors use this test to detect De Quervain’s tenosynovitis. It stretches the thumb tendons sharply. Pain on the thumb side of the wrist indicates a positive result.

Use this modern 3-step version. It starts gently. Then it increases the stretch. Finally, it performs the classic move.

Step 1: Gravity-Assisted Ulnar Deviation Rest your forearm on a table. Let your hand hang off the edge naturally. Keep thumb pointing up. Relax completely. Allow gravity to pull your wrist toward the pinky side. Check for pain near the bump on the thumb side of the wrist.

Step 2: Gentle Passive Pressure If Step 1 causes no pain, apply light pressure. Push your hand further toward the pinky side. Keep the thumb relaxed. Look again for sharp pain over the tendons.

Step 3: Classic Finkelstein Maneuver Make a fist. Tuck your thumb inside your fingers. Close your hand tightly over the thumb. Now bend your wrist toward the pinky side. Perform the movement actively first. Then let someone gently guide or add pressure if needed.

Positive Sign Sharp pain flares at the base of the thumb. It strikes the area near the wrist bump on the thumb side. Pain often spreads along the tendons. Compare both wrists – the affected side hurts much more.

Perform it slowly. Stop immediately if pain becomes severe. This test reproduces your symptoms. Consult a doctor for accurate diagnosis and proper treatment.
