Laboratory parameters in hyperosmolar hyperglycemic non-ketotic coma

In this article we will discuss the Laboratory parameters in hyperosmolar hyperglycemic non-ketotic coma

In this article, we will discuss the Laboratory parameters in hyperosmolar hyperglycemic non-ketotic coma. So, let’s get started.

Laboratory parameters

  • Plasma glucose (mg/dl)= >600 (600-1200 mg/dl)
  • Arterial pH= osmolality >7.30
  • Serum bicarbonate (mEq/L)= >15 mEq/L
  • Effective serum osmolality (mOsm/kg)= >350 (350-800 mOsm/kg)

Note: The effective serum osmolality is calculated by= 2x Na(mEq/L)+ glucose divided by 18 (mg/dl)+ BUN/2.8

  • Anion gap [(Na+) + (K+)] – [(Cl-) – (HCO3-)]= 20
  • Urine or serum ketone= Small or absent
  • Blood urea/creatinine= May be high (pre renal azotemia)
  • Serum sodium (mEq/L)= 135-145 mEq/L
